DIGI-CARE (Strengthening DIGItal technologies for enhancing the Continuum of CARE for Diabetes and Hypertension Management) is an implementation research study under the ICMR National Health Research Priority initiative. The project aims to co-develop, implement, and evaluate a health system model that improves the adoption and effective use of existing digital health technologies for diabetes and hypertension management under the NP-NCD programme. By strengthening digital infrastructure, provider competency, workflows, and data use, the project seeks to improve the continuum of care for non-communicable diseases within the Indian public healthcare system.

DIGI-CARE is a multi-site, multi-institutional project implemented across districts in Assam, Chattisgarh, Himachal Pradesh, Kerala, Puducherry, Solan with the Centre for Chronic Disease Control leading implementation in Solan district, Himachal Pradesh.
The project is carried out in three phases over three years. In the first phase (Formative), the team studies the current digital health systems, identifies challenges, and understands the needs of healthcare providers and patients. In the second phase (Co-design and model optimisation), healthcare providers, government officials, and community members work together to design and improve solutions through pilot testing and feedback. In the final phase (Implementation), the improved model is implemented across all study districts and evaluated for its effectiveness.
The project focuses on improving digital infrastructure, training healthcare providers, strengthening digital support systems, redesigning workflows, and promoting better use of digital health technologies in public healthcare facilities.
